Ketogenic diets for dogs

An overview of ketogenic diets for dogs with epilepsy and cancer, explaining potential benefits, current evidence, and why veterinary supervision and monitoring are essential.

What is a ketogenic diet?

A ketogenic diet is a dietary approach characterized by very low carbohydrates, moderate protein, and high fat. The goal is to shift the body into ketosis — a metabolic state in which fat (and ketone bodies) are used as the primary energy source instead of glucose. In dogs, ketogenic diets are mainly discussed in two contexts: epilepsy and cancer. However, the scientific evidence is still variable, and this is not a “do-it-yourself” intervention without professional supervision.

Ketogenic Diet and Epilepsy

Some studies in dogs with idiopathic epilepsy suggest that diets high in MCT fats (medium-chain triglycerides) can reduce seizure frequency in certain dogs. Not all dogs respond, but some experience fewer and milder seizures. Possible mechanisms include: - Stabilization of blood glucose. - Increased production of ketones. - Direct effects of MCTs on brain energy metabolism. Berk et al. found that several dogs experienced a reduction in seizure frequency following supplementation with MCT oil. This supports earlier studies showing that medium-chain fatty acids (caprylic acid C8 and capric acid C10) have a direct antiepileptic effect. They also observed changes in lipid, amino acid, and ketone metabolism in the dogs, indicating that MCTs influence energy metabolism in ways that may benefit brain function and neurological health. The dogs received a specific MCT oil, and the dose corresponded to 9% of their daily metabolizable energy intake. However, responses vary between individuals. Some dogs show clear improvement, while others experience little or no measurable benefit.

Ketogenic Diet and Cancer

The rationale here is based on the so-called Warburg effect: many cancer cells rely more heavily on glucose than healthy cells. The theory is that lowering blood glucose and increasing ketone levels may place metabolic stress on cancer cells, while healthy cells can adapt more efficiently to using fats and ketones for energy. There are promising results from animal models and some clinical observations, but evidence in dogs remains limited. Therefore, ketogenic diets should be considered a potential supportive strategy, not a replacement for veterinary cancer treatment.

What Characterizes a Homemade Ketogenic Diet?

A therapeutic ketogenic diet must be carefully formulated. A typical energy distribution may be: - 70–85% of calories from fat - 15–25% from protein - 0–5% from carbohydrates This is different from simply feeding a “carbohydrate-free raw diet.” Protein often needs to be moderated, and fat intake intentionally increased to achieve actual nutritional ketosis. In addition, the diet must still be nutritionally complete, providing appropriate levels of vitamins, minerals, and essential fatty acids.

Important: This Should Be Done with Veterinary Guidance

A ketogenic diet significantly alters metabolism. Therefore, it should always: - Be implemented in consultation with a veterinarian - Include regular blood tests (liver values, lipids, electrolytes, etc.) - Include ketone monitoring (blood or urine) - Be accompanied by careful monitoring of body weight, appetite, and overall health Some dogs are not suitable candidates for ketogenic diets, for example those with pancreatitis, severe liver disease, or certain metabolic disorders.

A ketogenic diet may be an interesting tool for managing epilepsy and potentially as supportive care in cancer. However, the scientific evidence is still evolving, and individual responses vary. This approach should be viewed as metabolic therapy — not simply a low-carbohydrate diet. When properly formulated and carefully monitored, it may serve as a useful supplement. Without professional guidance, however, it may carry risks. For this reason, ketogenic diets for dogs should always be implemented together with a veterinarian and with systematic monitoring throughout the process.
